Choosing home exercise program (HEP) software comes down to a few questions: how you want to build programs, how your patients will receive them, whether you need RTM or an EMR integration, and what you can spend. The tools differ more than their marketing suggests.
We make ProperForm, one of the tools below, so we've tried to make this comparison useful rather than flattering. Every price and feature for the other tools comes from that vendor's own website as of October 2026. Prices change, so confirm on the vendor's site before you buy.

What to look for in HEP software
- How programs get built. Most tools start from a stock library of thousands of exercise videos. A few let you film your own. ProperForm builds the program from video of the patient you're treating.
- How patients receive it. A patient app with reminders is the norm now. Some tools still lean on printouts and PDFs.
- RTM. If you bill remote therapeutic monitoring, check whether RTM is included, an add-on, or missing.
- EMR fit. Some HEPs come bundled with an EMR, some integrate with several, and some stand alone.
- Price model. Per provider per month, per year, per clinic, or by active patient.
The tools
ProperForm
ProperForm takes a different approach from library-based tools. You record your patient doing each exercise during the session and talk through your cues as you normally would. ProperForm AI turns what you said into a structured program, with names, parameters and cues for you to review and edit, in about three seconds. The patient scans a QR code and gets their program, with video of themselves, in the ProperForm app.

- Pricing: Health Free ($0, 5 programs a month, up to 3 exercises per program, 60-second recordings). Health Solo $24.99/month for one clinician with unlimited patients, programs and exercises. Health Team $29.99 per provider/month adds patient sharing across the team. Annual billing is about 10% less.
- Platforms: Clinicians record on iPhone, iPad or Android phone and can view programs on the web. Patients use iOS or Android.
- Worth knowing: HIPAA compliant. There's no stock exercise library, by design. RTM and EMR integrations are planned for Health Enterprise (coming late 2026); today you copy a program summary into your EMR.
MedBridge
MedBridge pairs a HEP with a large continuing education catalog, and sells to everyone from individual clinicians to health systems.
- Pricing: Individual plans are annual, paid upfront, and only the Premium plan can share HEPs with patients. Small business (2+ seats) and enterprise pricing is by quote.
