What is ProperForm?

1. What is ProperForm?

ProperForm is a video-based platform that helps clinicians create personalized home exercise programs (HEPs) in just a few minutes. Physical therapists, occupational therapists, speech-language pathologists, chiropractors, and any other type of rehabilitation therapy provider can record patients performing exercises during an appointment. ProperForm AI then summarizes the clinician’s cues and coaching points to create a modern, easy-to-follow program patients can use at home.

2. Who can use ProperForm?

ProperForm is designed to improve the home exercise program (HEP) experience for both clinicians and patients. Patients can use ProperForm when their provider has an account. Although ProperForm was built primarily for physical therapy, it can also support rehabilitation hospitals, occupational therapists, speech-language pathologists, chiropractors, personal trainers, and other professionals who prescribe at-home exercises.

3. Why should I use ProperForm?

ProperForm helps clinicians create personalized home exercise programs (HEPs) during the appointment using the coaching they already provide. Patients receive familiar video demonstrations, individualized cues, and clear instructions they can revisit at home. Clinicians spend less time documenting programs after appointments while giving patients a more engaging at-home experience.

4. Can ProperForm really save me time?

ProperForm is designed to save clinicians an average of eight minutes per patient potentially an hour or more each day. By recording the instruction already happening during an appointment, clinicians can build the patient’s home exercise program (HEP) as part of the visit instead of creating it afterward.

Technology, Security & HIPAA Compliance

1. Are there separate apps for patients and clinicians?

No. ProperForm uses one app for both patients and clinicians. When you sign in, the app automatically recognizes your account type and displays the appropriate experience.

2. What devices does ProperForm support?

  • Clinicians can use an iPhone or iPad running iOS 18 or later to create and manage programs, with web browser access for viewing programs and easily copying details into your EMR.
  • Patients can use supported Apple or Android devices.

3. Our clinic has only one iPad. How can multiple clinicians use ProperForm?

We recommend using clinic-owned devices when possible, but clinicians may also sign in to ProperForm on their personal Apple devices. We also support multiple clinicians on a single device but each provider must use their own account.

4. What if my clinic does not have a reliable internet connection?

Your patient is only in front of you once, so recording and sharing never require an internet connection. Capture their exercises and share the program while they’re still with you, even offline. Once you reconnect, AI fills in the program details so you can review and publish. Your patient will automatically receive a push notification when their program is ready.

5. Why does my device need a passcode or biometric authentication?

ProperForm is a HIPAA-compliant platform designed to protect sensitive patient information. Requiring a device passcode, Face ID, or other biometric authentication allows ProperForm to securely encrypt and protect patient data stored on your device.

6. What happens if my device is lost or stolen?

ProperForm protects patient information using your device’s passcode or biometric authentication, such as Face ID or fingerprint unlock. If your device is lost or stolen, your ProperForm data remains protected from anyone who can’t authenticate as you. Contact ProperForm Support and your organization’s administrator promptly so any additional security measures can be taken.

7. Is ProperForm HIPAA compliant?

8. How long does ProperForm retain health data?

Data retention periods vary by plan. Check with your provider or organization to learn how long your information is retained under their ProperForm plan.

9. What happens to my data if I stop using ProperForm?

If your organization stops using ProperForm, it can download its patient data before the end of service. ProperForm also offers secure data-storage options that allow former customers to retain and request access to their data after service ends. Contact ProperForm Support for available options and instructions.

10. How does ProperForm protect our information?

ProperForm uses industry-standard security practices designed to protect sensitive patient information and support HIPAA compliance. Data is encrypted both in transit and at rest, with access controls and authentication safeguards designed to prevent unauthorized access.

How to Use ProperForm

1. How should I introduce ProperForm to my patients?

Our Getting Started video walks you through how to use ProperForm, including how to introduce it to your patients and incorporate it naturally into an appointment.

You can also request a ProperForm Clinic Kit, which includes two tabletop signs for your front desk or treatment areas. The signs introduce patients to ProperForm and encourage them to download the app before their appointment, although downloading it ahead of time is not required.

2. What should I do if my patient does not want to be recorded?

That’s okay. Before recording, explain how the video will be used, who will be able to access it, and how ProperForm protects their information. Always follow your organization’s consent and privacy policies.

If your patient still prefers not to be recorded, you can point the camera away and record only your spoken exercise instructions. ProperForm AI can use your instructions to help build their program without capturing the patient on video.

3. How do I create a home exercise program?

Watch our Getting Started video for a quick walkthrough, or follow these simple steps:

  1. Tap the purple + button in the lower-right corner.
  2. Tap Record New Exercise.
  3. Start recording.

For the best results, demonstrate the exercise first and begin recording once your patient is performing it with the correct form. Keep each video around 20–30 seconds and clearly say the exercise name, along with the cues, sets, reps, frequency, weight, or resistance your patient will need at home.

Focus on the coaching that matters most. ProperForm AI will use what you say during the recording to turn your cues and prescription into structured exercise instructions.

4. Can ProperForm provide printed instructions?

Yes. For patients who prefer paper, ProperForm can generate a personalized PDF handout using still images from their own exercise videos along with their exercise instructions.

To print a program:

  1. Tap Share Program at the top or bottom of the screen.
  2. Enter the patient’s date of birth.
  3. Tap Printable Program to have ProperForm generate a PDF.
  4. Print directly using AirPrint, or securely send the PDF to yourself or an authorized staff member for printing.

5. How many exercises can I include in one program?

There is no set limit. Include the number of exercises appropriate for the patient and their recovery plan.

6. Should I create a new program each time I see a patient?

Usually, no. If you’re continuing to treat the same condition, update the patient’s existing program instead:

  1. Search for the patient by name.
  2. Open their most recent program.
  3. Tap the pencil icon and select Edit Program.
  4. Remove exercises they no longer need, record new ones, and rearrange the program as needed.

For a new condition or injury, you can create a separate program or update the existing one based on what makes the most sense for the patient.

7. How long can a patient access a program?

Patients can access their programs as long as the clinic remains a ProperForm customer, the program has not been deleted, and it remains within the clinic’s data retention period.

Data retention varies by plan and organization. Check with your provider or organization administrator for details about how long your programs will remain available.

8. How do I share a program with a patient?

When you’re ready to share:

  1. Tap Share Program.
  2. Enter the patient’s date of birth.
  3. Have the patient scan the QR code, or send the program by text or email.
  4. If the patient prefers paper, select Printable Program to generate a personalized PDF.
  5. When the program is ready, tap Publish. Your patient will automatically receive a push notification letting them know their program is ready.

9. What if my patient has not downloaded the app?

That’s okay. Your patient can still scan the QR code, which will guide them to download the ProperForm app. Once they’re set up, their program will be waiting for them.

10. What if my patient goes home without downloading the app?

That’s okay. You can send their program by text or email at any time. If they received a printed program, they can also scan the QR code on the handout to download ProperForm and access their program.

ProperForm AI

1. How long does ProperForm AI take to generate exercise instructions?

With a typical network connection, ProperForm AI generates exercise instructions in just a few seconds. Processing may take a little longer with slower connections or significant background noise.

2. Do I need to review instructions generated by ProperForm AI?

Yes. ProperForm AI does the busywork, but you’re always in control. Before publishing, review the exercise title, instructions, sets, reps, and other prescription details for accuracy.

You can quickly make changes by typing or using voice input. As the treating clinician, you review and approve the final program before it’s published to your patient.

3. ProperForm AI did not generate my instructions. What should I do?

Don’t worry, you don’t need to record your patient again. Try these steps:

  • Make sure your device is close enough to clearly capture your voice. In a noisy clinic, keep it closer to you while recording.
  • Tap the ProperForm AI button, the purple circle in the lower-left corner, to process the recording again.
  • If instructions still aren’t generated, open the exercise instructions and record your coaching cues again using voice input.
  • You can keep working while ProperForm AI processes in the background. Tap the purple + button to move on to your next exercise.

EMR Integrations

1. Does ProperForm integrate with my EMR?

ProperForm is designed to work alongside your existing EMR without requiring an integration. AI-generated program summaries can be copied from ProperForm and pasted directly into the patient’s chart.

We’re also working with select EMR platforms on deeper integrations to make this workflow even more seamless.

2. How do I use ProperForm with my EMR?

ProperForm gives you two summaries you can easily copy and paste into your EMR from iPhone, iPad, or the web:

  • Clinical Summary: An AI-generated clinical overview of the patient’s program, ready to copy into your documentation.
  • Full Summary: A complete summary containing all program and exercise details.

3. Which EMR integrations are coming to ProperForm?

We’re actively working with select EMR platforms to build deeper integrations with ProperForm. These integrations are being designed to reduce duplicate work and make it easier for ProperForm to fit naturally into your existing clinical workflow.
